there is a two-step process on submitting your information for a damage assessment. 

You must complete both steps for our reporting process.

Step One: Fill out the Initial Disaster Intake/Assessment Form on the Indiana 211 website by clicking here

Step Two: Upload 1 to 5 images of your flood damage by clicking here
